چکیده

309 Difficult-to-treat Asthma with Idiopathic Chronic Eosinophilic Pneumonia Norbert Pauk, PhD, MD. Department of Pneumology, Faculty Hospital Na Bulovce, 3rd Faculty of Charles University, Prague, Czech Republic. Background: Idiopathic chronic eosinophilic pneumonia (ICEP) is a rare disorder of unknown cause characterised by subacute or chronic respiratory and general symptoms, alveolar and /or blood eosinophilia, and peripheral pulmonary infiltrates on chest imaging. Interestingly, some but not all patients diagnosed with ICEP have a history of asthma, whilst others may develop asthma after a dignosis of ICEP has been made. Methods: We present this rare and interesting case, because ICEP is a rare complication of asthma, although it is seldom mentioned in reviews ant textbooks on asthma. Asthma in patients with ICEP is relatively severe and get worse after diagnosis of ICEP. Results: A 70-year-old woman with a history of asthma and chronic rhinitis with polyps (diagnosed in 2003), nonsmoker, history of allergies negative. She suffered from frequent exacerbations of asthma (7 times a year with repeated courses of oral corticosteroids). In 2006 she had sudden fever, weight loss, malaise and impaired dyspnea with productive cough, mild chest pain on sternum and respiratory failure. A chest radiograph demonstrated bibasilar infiltrates. Peripheral blood smear showed a newly developed, marked eosinophilia, and a chest X-rays and HRCT scan revealed a diffuse patchy nodular infiltrate in all lung fields. Serum-precipitating antibodies against Aspergillus antigens negative, no cutaneous reactivity to Aspergillus antigen, negative findings for parasitic infections, no central bronchiectasis on previous HRCT, ANCA, ANA, ENA negative. She received an intensive course of corticosteroids with complete resolution of symptoms and the eosinophilia, as well as decreased infiltrates on chest radiograph. Doses of corticosteroids slowly reduced a maintained until June 2009. Her asthma often exacerbates so far and needs intermittent courses of corticosteroids, is difficultto-treat, but without any relapses of ICEP. Conclusions: Clinicians should consider pulmonary eosinophilia in the differential diagnosis of patients treated for asthma who develop pulmonary infiltrates with dyspnea.

A study on the esophageal cancer radiotherapy effects on the patient’s lung health

Introduction:   Radiotherapy with or without surgery plays an important role in the treatment of patients with esophageal cancer. In the treatment planning of esophageal cancer, usually normal lung volume was observed in the treatment fields and probably received high radiation dose. The incidence of radiation pneumonitis (RP) after radiotherapy (RT) for sensitive organ of lu...

Investigating of relationship between the parameters related to the treatment planning and the biological parameters in radiation induced pneumonitis in breast cancer patients

Introduction: The purpose in this study is to analyze of DVH parameters as predictors of radiation pneumonitis and investigation on the relationship between dosimetric and clinical parameters based on PFTs as response in NTCP model and therefore achieve to lung dose- response relation in breast cancer patients. Prescribed dose to treatment volumes was 50 Gy in 2.0 Gy/fractions ...

Predictive values of TNF-α, IL-6, IL-10 for radiation pneumonitis

Background: To investigate the expression of TNF-α, IL-6, IL-10 in the plasma of patients with lung cancer who received radiation therapy (RT), and to analyze the correlation between these cytokines and radiation pneumonitis (RP). Materials and Methods: Patients with lung cancer who received 3D-CRT in our hospital were prospectively evaluated. Circulating cytokine levels were measured wit...

Comparing of different normal tissue complication probability models for plan evaluation of radical radiotherapy for esophageal cancer

Introduction: Chemoradiation is a well-established protocol for management of localized esophageal cancer. However, some early and late toxicities are associated with this therapy. Plan evaluation in term of normal tissue complication probability (NTCP) may help clinicians to tailor this therapy more individually. Given the variety of present radiobiological models, the choice...

Curcumin Mitigates Radiation-induced Lung Pneumonitis and Fibrosis in Rats

Radiation-induced lung injury is one of the most prominent factors that interfere with chest cancer radiotherapy, and poses a great threat to patients exposed to total body irradiation. Upregulation of pro-oxidant enzymes is one of the main mechanisms through which the late effects of ionizing radiation on lung injury can be exerted. Interleukin (IL)-4 and IL-13 are two important cytokines that...
